What to expect during Pleurx Catheter Placement (chest)

-There are both chest & abdominal Pleurx Catheters. We did a chest catheter placement.
-Position: Supine.
-Packs, Drapes & Gowns: General pack, 2 gowns, a lap drape & utility drapes (or you could towel off with 4 blue towels).
-Instruments: Pleurx Catheter Kit (which comes with the local, a long 2-0 Silk needle, a #11 retractable safety scalpel & 3 hypos you’ll have to count, along with the tunneler, guidewire, etc. ), a 6 in. needle driver, curved Mayo scissors & Adson without teeth. I grabbed a probe cover to hold in case we needed it, which we did.
-Suture: 4-0 Monocryl y426 (for skin) & 3-0 Ethilon 1663 (to tie in the catheter)
THE PROCEDURE:
1- Local (1% Lidocaine Plain)
2- Incision (Green #11 retractable safety scalpel – hand upsidedown)
3- Spinal needle on 10 mL syringe to make puncture, pull fluid & slide sheath forward
4- Advance the guidewire
5- Attach the metal tunneler to the catheter
6- Make another stab incision
7- Insert the tunneler attached to the catheter
8- Remove tunneler
9- Place the “T” shaped sheath onto the wire, advance, then feed the catheter inside of it
10- Peel away the sheath – the surgeon may ask you to pull one side for them at the same time as they pull the other side
11- Nylon suture to suture the catheter in place
12- Drain (our patient drained 8 canisters of fluid so we had to wait 35+ minutes, this may not be the case for all patients. Ours was really full of fluid.)
13- Dressings
