Anterior and Posterior Repair (Colporrhaphy)

Working through the vagina, the surgeon tightens the weakened tissue between the vagina and the bladder in front and between the vagina and the rectum behind, and trims the stretched vaginal lining, so the bladder and rectum no longer bulge into the vagina.

The case

An anterior and posterior repair (anterior and posterior colporrhaphy, or "A and P repair") fixes two kinds of vaginal prolapse in one operation. A cystocele is the bladder bulging into the front wall of the vagina. It can cause pressure, a bulge at the vaginal opening, trouble emptying the bladder or stress incontinence. A rectocele is the rectum bulging into the back wall of the vagina. It can cause a bulge, pressure and difficulty passing stool. An enterocele, a small hernia of bowel through the top of the vagina, may be present too. Childbirth is the most common cause; aging, an inherited weakness or past surgery can also play a part. The whole operation is done through the vagina, with no abdominal incision. The anterior repair opens the front wall, frees the bladder and gathers the fascia beneath it with plication stitches, sometimes with a Kelly stitch at the bladder neck for incontinence. The posterior repair opens the back wall, frees the rectum, closes the fascia over it, may bring the levator muscles together, and usually rebuilds the perineum (perineorrhaphy). Either repair can be done alone, and both are often added to a vaginal hysterectomy. Current evidence does not support using mesh through the vagina for rectocele repair. For the scrub, this is a vaginal-tray case done with the patient in lithotomy and the surgeon seated. It uses many Allis and Allis-Adair clamps and long rows of plication sutures. The anterior half is finished, the instruments are reset, and the same pattern is repeated on the back wall. Possible complications are bleeding or hematoma, urinary retention or infection, a narrowed or shortened vagina, painful intercourse, injury to the bladder or rectum, and the bulge coming back.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
