DIEP Flap Breast Reconstruction

Skin and fat from the lower abdomen are moved to the chest on the deep inferior epigastric perforator vessels, which are reconnected to the internal mammary vessels under the microscope to rebuild the breast without taking the rectus muscle.

The case

A DIEP (deep inferior epigastric perforator) flap rebuilds a breast after mastectomy using the patient's own lower-abdominal skin and fat. It is a free flap: the tissue is completely detached and its blood supply is reconnected in the chest with microsurgery. Only the small perforator vessels and the deep inferior epigastric artery and vein come with the flap, sometimes with a small piece of the rectus muscle around the perforator. The muscle itself stays in the abdomen, and this is the main difference from a TRAM flap. Because the muscle is spared, the abdominal wall has much less damage, and in many centers DIEP and SIEA flaps have replaced the free TRAM as the first choice for breast reconstruction with the patient's own tissue. The operation has two parts that two surgeons can do at the same time: raising the flap on the abdomen and preparing the recipient vessels in the chest, which are most often the internal mammary vessels. The artery is usually sewn by hand under the microscope and the vein is often joined with a coupler. A rebuilt breast made from the patient's own tissue feels soft, ages naturally and changes with body weight, and it avoids implant problems such as capsular contracture. The abdominal donor site is closed much like an abdominoplasty, which also contours the abdomen. The case is long and needs an experienced microsurgeon. The most common problems are wound problems such as infection, seroma, hematoma and delayed healing. Clotting in the connected vessels can cause loss of the flap, and fat necrosis can occur. At the donor site, an abdominal bulge, hernia or weakness can develop. Obesity, smoking, prior abdominal surgery and clotting disorders raise the risk; a prior abdominoplasty rules out a DIEP flap.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
