Latissimus Dorsi Flap

The latissimus dorsi muscle, with or without a skin island, is lifted off the back on its thoracodorsal vessels and turned through a tunnel under the armpit to rebuild the breast or cover a chest defect.

The case

A latissimus dorsi flap moves the broad, flat latissimus muscle of the back, often with an island of the skin over it, to another part of the body. The muscle is fed by the thoracodorsal vessels, which run along its deep surface. Its most common use is breast reconstruction after mastectomy, when there is a large tissue loss or when a TRAM flap is not an option. It is also used to cover large defects of the chest, head and neck, and as a muscle transfer for facial reanimation. For the breast, the flap is usually pedicled: it stays attached to the upper arm bone and to its own blood vessels and is swung through a tunnel under the armpit to the front of the chest. To make a more natural breast mound, a breast implant is usually placed under it. The flap can also be moved as a free flap, where the vessels are cut and reconnected at the new site with microsurgery. What makes this case different for the scrub is the position change: the flap is harvested with the patient on the side (or prone), the back is closed, and the patient is turned supine to finish the chest. The operation is done under general anesthesia, and blood loss can be significant, so a type and screen is done beforehand. The most serious complication is partial or complete flap loss from poor blood flow in the first weeks of healing. Bleeding and hematoma at the back or chest are prevented with careful hemostasis. Prior injury or surgery that damaged the flap's blood supply rules it out; smoking, diabetes and poor vascular health raise the risk.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
