Repair of Atrial Septal Defect (ASD)

The abnormal opening in the wall between the right and left atria is closed with sutures or a patch through an opened right atrium, stopping blood from shunting from the left side of the heart to the right.

The case

An atrial septal defect (ASD) is a hole in the wall between the two upper chambers of the heart. Oxygen-rich blood leaks from the left atrium into the right, so the right side of the heart and the lungs handle too much blood. Many small defects close on their own in the first year of life, but a large defect can over time enlarge the right heart, raise the pressure in the lungs and lead to right heart failure, rhythm problems or stroke. The three main types are ostium secundum (the most common, in the middle of the septum), sinus venosus (high, near the superior vena cava, often with abnormal pulmonary veins) and ostium primum (low, often with a cleft mitral valve). Many secundum defects are now closed with a device in the cardiac catheterization lab. Surgery is used for the other types and for defects that are not suitable for a device. Through a median sternotomy (or occasionally a right anterolateral thoracotomy or another small incision), the child is placed on cardiopulmonary bypass, the right atrium is opened, and the hole is either sewn shut or covered with a patch of Dacron or the child's own pericardium. The setup is the same as an adult open-heart case with smaller instruments. Small patients cool quickly and have little blood to lose, so warming and careful tracking of blood loss and irrigation are part of the scrub's job. Risks include air embolism, rhythm problems or heart block, bleeding, infection and a leftover leak around the patch.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
