Repair of Tetralogy of Fallot (TOF)

The ventricular septal defect is closed with a patch and the narrowed right ventricular outflow tract and pulmonary valve are opened and widened, so the right ventricle can pump blood freely to the lungs and only oxygenated blood reaches the aorta.

The case

Tetralogy of Fallot is the most common cyanotic ("blue baby") heart defect. It has four parts: a hole between the ventricles (VSD), narrowing at or below the pulmonary valve that blocks blood leaving the right ventricle, an aorta that sits over the VSD, and a thickened right ventricle. Because blood has trouble reaching the lungs, oxygen-poor blood crosses the VSD into the aorta, and the child may have sudden spells of deep cyanosis (tet spells). Without repair, few patients survive to adulthood. Complete repair closes the VSD with a patch and opens the right ventricular outflow tract by cutting away obstructing muscle, opening a fused pulmonary valve and, when needed, widening the outflow tract or the pulmonary valve ring with a patch. It is done through a median sternotomy on cardiopulmonary bypass, most often in the first year of life. Some infants first have a palliative shunt between a systemic artery and the pulmonary artery to increase lung blood flow until full repair. Surgeons increasingly work through the right atrium and pulmonary artery to avoid a large cut in the ventricle, and some anatomy requires a conduit from the right ventricle to the pulmonary artery. The setup is an open-heart setup with pediatric-sized instruments and several patches. Risks include heart block from stitches near the conduction system, bleeding, residual obstruction or leak, rhythm problems and infection. Warming, accurate tracking of irrigation and blood loss, and patch materials ready on time are key for the scrub.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
