Robotic-Assisted Prostatectomy with Pelvic Lymph Node Dissection

The lymph nodes along the pelvic sidewalls are removed for staging, then the whole prostate and seminal vesicles are taken out with the da Vinci robot and the bladder neck is joined to the urethra.

The case

In prostate cancer, removing the pelvic lymph nodes is the most reliable way to find out whether the cancer has spread, because scans often miss small nodal disease. When the risk of spread is high enough, the surgeon adds a pelvic lymph node dissection (PLND) to the radical prostatectomy, and "possible" in the booking reflects that the node dissection is done when it is indicated. Nodes may be sent for frozen section, and positive nodes can change how much is removed or what treatment follows. Men with low-risk cancer are often spared the dissection. The extent of the dissection varies. A limited template takes the nodes in the obturator fossa, often with those over the external iliac vessels. An extended template adds the nodes over the internal iliac (hypogastric) and common iliac vessels and can yield about 13 to 24 nodes. In the robotic operation on this page the node dissection is done first on each sidewall, with clips on the lymph channels, and the prostatectomy follows a posterior-first laparoscopic sequence: the vasa and seminal vesicles are freed from behind before the bladder is dropped forward. The page "Robotic-Assisted Radical Prostatectomy" covers the operation without the node dissection. The main extra risk of the node dissection is a lymphocele, a collection of lymph fluid in the pelvis, which happens in roughly 3 percent of robotic cases and occasionally needs drainage. Injury to the obturator nerve or iliac vessels is rare. After laparoscopic surgery, the drain and catheter come out on about the third day, and a blood thinner is prescribed for two weeks.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
