Thoracotomy

An incision is made through the chest wall between two ribs, a rib is removed if needed, and a rib spreader opens the space so the surgeon can reach the lungs, heart, esophagus or great vessels; at the end the ribs are drawn back together and the chest is closed around chest tubes.

The case

A thoracotomy is an incision through the chest wall into the chest cavity. It is the open route to the lungs, esophagus, the descending aorta and sometimes the heart, and the steps for opening and closing the chest are much the same whatever operation is done inside. Lung cancer and other lung diseases, esophageal disease, aortic problems and injuries are common reasons. Many lung operations are now done by VATS, but a thoracotomy is still used when open exposure is needed or a VATS case has to be converted. The most common incision is the posterolateral thoracotomy, done with the patient on the side: it curves from under the breast around the tip of the shoulder blade, and the chest is entered through an intercostal space. Anterolateral incisions above or below the breast are done with the side tilted up, and a muscle-sparing vertical incision under the arm avoids cutting the big back muscles. A rib is occasionally removed for more room. The ribs are spread with a Finochietto retractor and, at the end, brought back together with heavy pericostal sutures and a rib approximator. Bleeding (especially from the vessels under each rib), infection, air leaks and fluid in the chest, shoulder stiffness and long-lasting pain along the incision are the main problems after a thoracotomy. Chest tubes connected to a water-seal drainage unit are left in to drain air and fluid.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
