Thymectomy

Through a median sternotomy, the thymus gland and the fat around it are dissected off the pericardium and great veins in the anterior mediastinum and removed.

The case

Thymectomy is removal of the thymus gland, which sits in the front of the chest behind the sternum, on top of the pericardium and the great veins. The thymus is large in infants and shrinks and turns to fat after puberty. The operation is done to remove tumors of the thymus (thymoma and thymic carcinoma) and to treat myasthenia gravis, an autoimmune disease of muscle weakness that is closely linked to the thymus. The standard approach, and the one walked through here, is a median sternotomy: the sternum is split with a saw or sternal knife, and the whole gland is removed together with the fat around it, from the innominate vein down to the diaphragm and out to both phrenic nerves. Smaller tumors and myasthenia gravis are often treated through less invasive approaches instead: a small incision in the lower neck (transcervical), VATS, or robotic surgery. Bleeding from the thymic veins or the innominate vein, injury to the phrenic nerves, the heart or the pericardium, air leaks and chylothorax are the main risks. Patients with myasthenia gravis are stabilized before surgery and may have breathing problems afterward, so the drugs used to treat the disease and a tracheostomy tray should be at hand.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
