Transvenous Lead Extraction

Pacemaker or defibrillator leads that have scarred into the veins and heart are freed and pulled out through the vein they entered, using locking stylets, telescoping dilator sheaths, powered mechanical or laser sheaths and, if needed, a snare from the femoral vein, with surgery on standby.

The case

Transvenous lead extraction (TLE) removes pacemaker and ICD leads from inside the veins and heart without opening the chest. The main reasons are infection of the device pocket or leads (including vegetations on the leads or valves), broken or failed leads, and some system upgrades. Leads that have been in for years become encased in dense scar along the subclavian and innominate veins, the superior vena cava and the heart wall, and simply pulling on them can tear these structures; leads implanted for more than 10 years are especially difficult. The operation follows a stepwise plan, moving to the next tool only when the previous one fails: simple traction with a standard stylet, then a locking stylet that grips the inside of the lead, then telescoping non-powered dilator sheaths passed over the lead to break up the scar, then powered sheaths (a mechanical rotating sheath with a cutting tip, or an excimer laser sheath), and finally a snare passed up from the femoral vein for broken pieces. The sheath is advanced to the heart wall and held there (countertraction) while the lead is pulled, so the heart is not pulled inward. Serious complications occur in about 1 to 3% of cases: tearing of the superior vena cava or innominate vein, or perforation of the atrium or ventricle, which causes sudden massive bleeding or tamponade and needs immediate surgery. For this reason the case is done in an OR or hybrid lab with an anesthesia provider, a cardiac surgeon and a heart-lung machine ready, echo to watch for pericardial fluid, and often a balloon that can be inflated to block a torn superior vena cava. The scrub person must be ready to open for an emergency sternotomy at once.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
