Venous Thrombectomy for Deep Vein Thrombosis

Clot is removed from the iliac and femoral veins through an opening in the common femoral vein in the groin, using balloon catheters and compression of the leg, to restore blood flow out of the leg.

The case

Deep vein thrombosis (DVT) is a blood clot in the deep veins, and when it fills the iliac and femoral veins (iliofemoral DVT) it can leave the leg chronically swollen and painful (post-thrombotic syndrome) or break off and travel to the lungs. Most DVT is treated with blood thinners alone or with catheter-based treatments done through a needle puncture. Open venous thrombectomy is used for selected patients with a large, fresh iliofemoral clot, for patients who cannot have clot-dissolving drugs, and for the most severe form, phlegmasia cerulea dolens, where the leg is so swollen and blue that it is at risk. This is a venous operation, which sets it apart from a femoral arterial embolectomy even though both are done through the groin with Fogarty balloon catheters. The surgeon opens the common femoral vein, sweeps clot out of the iliac veins and sometimes the vena cava, and pushes clot up out of the leg by squeezing or wrapping it. Because the clot can break loose toward the lungs, extra steps protect against pulmonary embolism: a blocking balloon in the vena cava when the clot reaches it, a high PEEP from the anesthesia team and a head-up tilt. A venogram checks the result, a stent may be placed in a narrowed iliac vein, and a small temporary arteriovenous fistula is often made to keep blood moving fast through the cleared vein. The case is often combined with endovascular work, so the room needs fluoroscopy, wires and stents. Wound infection and new clotting are the main risks afterward, and patients stay on blood thinners and compression stockings for months.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
