What it means
Developed by Barbara Braden and Nancy Bergstrom. It scores six areas: moisture, activity, mobility, nutrition, sensory perception, and friction and shear. Totals run from 6 to 23, and a lower score means a higher risk; many facilities treat 18 or below as at risk.
For the nurse
Score the client on admission, as often as policy requires and whenever their condition changes. Use the low subscores to plan care: turning schedules, moisture control, nutrition and pressure-relieving surfaces.
