consistency

How thick, firm or runny something is, such as stool, sputum, wound drainage or a food or drink.

kun-SIS-tun-see noun

What it means

In charting, consistency describes the texture of what the nurse sees: stool may be formed, soft, loose or watery; sputum may be thin or thick; a food or drink may be thin or thickened for a client who has trouble swallowing. In care planning the word has a second meaning: doing things the same way every time, such as staff applying the same rules or a client keeping a steady routine.

In a chart

"Large brown stool, soft consistency, no blood seen."
