What it means
A frequency used in orders and care plans. In charting it is shortened to q2h (q from the Latin quaque, every; h for hour). It is the classic interval for turning a client in bed and is common for checks such as neurovascular or vital sign checks after a procedure.
For the nurse
Turn and reposition a client who cannot move on their own at least every 2 hours to protect the skin, and record each turn.
In a chart
"Turned q2h, left side to back; skin over sacrum intact."
