What it means
An order frequency. "q" stands for "every" and "h" for "hour", so q1h is every hour, q4h every 4 hours and so on. ISMP warns that "qn" and "qhs" (nightly, at bedtime) are misread as "qh", which has led to drugs being given every hour by mistake, so those two should be written out.
For the nurse
If an order's frequency is unclear, clarify it with the prescriber before giving the drug.
