What it means
A behavioral pain tool for children from about 2 months to 7 years and for others who cannot say how much pain they have, such as some clients with cognitive impairment. The nurse watches the child and scores each of the five items 0, 1 or 2, for a total of 0 to 10. Higher scores mean more pain.
For the nurse
Watch the child for a few minutes, awake or asleep, use the same scale each time, and reassess after giving pain relief.
