What it means
On the fetal monitor, the heart rate slowly falls and then slowly returns to baseline after the contraction is over, so the dip lags behind the contraction. It is a concerning pattern because it is linked to uteroplacental insufficiencyyoo-ter-oh-pluh-SEN-tul in-suh-FISH-un-seeToo little blood flow between the uterus and the placenta for the fetus's needs.Full entry : during the contraction, the fetus is not getting enough oxygen. Repeated late decelerations, especially with poor variability, need prompt action.
For the nurse
Turn the client onto her side, stop any oxytocin infusion, give an IV fluid bolus and oxygen as ordered, check her blood pressure and notify the provider. Keep watching the pattern after each step.
