What it means
A legal record of the client's medications, usually electronic (eMAR). It lists scheduled and as-needed drugs and shows when each dose was given, held or refused, and by whom. With bar-code scanning, the nurse scans the client's band and each drug against the eMAR before giving it.
For the nurse
Compare the MAR with the order and the drug label, check the rights of medication administration, and record each dose right after giving it, never before. Record held or refused doses and the reason.
