What it means
An everyday word for stating a fact incorrectly, by mistake or on purpose. In nursing it comes up with documentation and handoffs: a misstated dose, time or allergy can lead to harm, and changing or falsifying a record is a legal and ethical violation.
For the nurse
If you find you have charted something wrong, correct it according to policy (in an electronic record, an amended entry); never delete or hide the original.
