What it means
OJ looks like OD or OS, the old abbreviations for the right and left eye. Medications meant to be mixed in orange juice have been put in a client's eye by mistake. ISMP lists OJ as error-prone: write "orange juice."
For the nurse
Write "orange juice" in full in orders and instructions.
