What it means

Used mostly for the peristomal skin, the ring of skin around an ostomy. Stool or urine leaking under the pouch, adhesive pulled off too roughly, or a pouch opening cut too large can make this skin red, sore and broken.

For the nurse

Check the peristomal skin at each pouch change. Cut the wafer opening to fit just around the stoma (about 1/8 inch, or 3 mm, larger), remove the old wafer gently, and clean and dry the skin before applying the new one.
