What it means
A standard order for giving information so that nothing important is left out, used when calling a provider, at handoff or when escalating a concern. Situation: what is happening now. Background: the history that matters. Assessment: what the nurse found and thinks is going on. Recommendation: what the nurse is asking for. ISBAR adds "Identify" at the start: who you are and who the client is.
For the nurse
Have the chart, the latest vital signs, allergies and medicines in front of you before you call. Read back any order you receive by phone.
In a chart
"S: Mr. Diaz, room 12, heart rate is now 128. B: Day 1 after hip surgery. A: He is pale and his blood pressure has dropped to 94/60. R: Please come and see him now."
