What it means
A short form for subcutaneous or subcutaneously. ISMP lists SC, SQ and "sub q" as error-prone: "SC" has been read as SL (sublingual), and "SQ" as "5 every". ISMP advises writing SUBQ, in capitals, or the full word.
For the nurse
Write SUBQ or "subcutaneous". If you receive an order with SQ or SC that is unclear, clarify it before giving the drug.
