suicide precautions

The safety measures used to protect a client who is at risk of harming or killing themselves.

SOO-ih-side prih-KAW-shunz noun

What it means

They include close observation, from checks every 15 minutes to a staff member staying within arm's reach at all times, depending on the level of risk. The room and belongings are checked and anything that could be used for harm is removed, such as belts, cords, shoelaces, sharp objects, plastic bags and some medicines. Meals may be served with safe utensils.

For the nurse

Ask directly about thoughts of suicide and a plan. Check that the client swallows medicines. Risk can rise when a severely depressed client starts to gain energy, so keep watching closely. Document observations at the ordered intervals.
