What it means

TAC has been used for several unrelated drugs: triamcinolone (a corticosteroid), tacrolimus (an immunosuppressant given after transplant), the topical anesthetic mix tetracaine-adrenaline-cocaine, and even a chemotherapy regimen. ISMP lists it as error-prone because one has been given in place of another.

For the nurse

Write each drug name in full, and clarify any order that says TAC.
