What it means
A swollen, raised patch of skin with a clear edge. Welts can come from an allergic reaction (hives), an insect bite, scratching or a blow from a narrow object. They often change shape or fade within hours.
For the nurse
Note the size, shape, place and number of welts, and ask what came before them. Welts with trouble breathing, throat tightness or swelling of the lips need emergency help. Welts in a pattern, such as the shape of a belt or cord, can be a sign of abuse and must be reported.
