What it means
Often written WNL. It tells the reader a finding is normal, but not what was actually checked, so many facilities ask nurses to chart the real findings instead or to use WNL only where their policy spells out what it means (charting by exception).
For the nurse
Chart what you saw, heard and measured ("lungs clear in all fields, no cough") rather than WNL alone, unless your facility's policy defines it.
