Abdominal Sacrocolpopexy

A prolapsed vaginal vault is lifted and held in place by mesh sewn to the front and back walls of the vagina and anchored to the ligament over the sacral promontory, through an abdominal incision.

The case

Sacrocolpopexy (also called sacral colpopexy) treats prolapse of the top of the vagina, most often after a hysterectomy, when the vault has dropped toward or out of the vaginal opening. Strips of synthetic mesh are sewn to the front and back walls of the vagina and the other end is fixed to the strong ligament on the front of the sacrum, so the vagina is held up in its natural direction. It is considered the gold standard for this kind of prolapse, with lower recurrence than repairs done through the vagina, at the cost of a longer operation and recovery. The operation can be done open (abdominal), laparoscopically or robotically with the same steps; this page covers the open abdominal approach. What makes it different from other pelvic surgery is that it works at two levels: an assistant pushes the vagina up from below with an EEA sizer or sponge stick while the surgeon sews mesh deep in the pelvis, and then the surgeon works directly on the front of the sacrum, next to the aortic bifurcation, the right ureter and the large veins of the pelvis. It is often combined with a hysterectomy, a cul-de-sac closure, a paravaginal or incontinence repair, and a vaginal repair at the end. The most feared problem during surgery is bleeding from the veins in front of the sacrum, which can be life-threatening. Bowel, bladder and ureter injuries and infection can occur, as in any abdominal surgery. The most common late problem is mesh wearing through the vaginal wall (erosion).

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
