Arterial Switch Operation

In a newborn with transposition of the great arteries, the aorta and pulmonary artery are cut and switched onto the correct ventricles and the coronary arteries are moved with the aorta, so the left ventricle pumps oxygen-rich blood to the body.

The case

In transposition of the great arteries (TGA), the two main arteries are switched: the aorta comes off the right ventricle and the pulmonary artery comes off the left ventricle. Blood then runs in two separate loops, one that never reaches the lungs and one that never reaches the body. A baby can survive only while some mixing happens through an open ductus or a hole between the chambers, so these babies are blue within the first days or weeks of life and need surgery early. The arterial switch operation puts the arteries back where they belong. Through a median sternotomy on cardiopulmonary bypass, the aorta and pulmonary artery are cut above their valves and swapped, the coronary arteries are cut out with a button of aortic wall and sewn into the new aorta, and the new pulmonary artery is rebuilt with a patch of the baby's pericardium. It has largely replaced the older atrial switch operations (Mustard and Senning), which redirected blood inside the atria but left the right ventricle pumping to the body. Surgeons differ in how they move the coronary arteries (open "trap-door" flaps, closed techniques, or reattaching them after the new aorta is filled), and in whether they use continuous bypass or short periods of circulatory arrest. The most serious early problem is poor blood flow through a moved coronary artery, which shows up as a struggling, poorly moving heart. Others include narrowing where the new pulmonary artery was joined, bleeding and, in swollen babies, the need to leave the chest open. For the scrub, this is a newborn open-heart setup with very fine instruments and 8-0 sutures, a large pericardial patch, and long, exacting steps.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
