Hallux Rigidus Surgery

The arthritic joint at the base of the big toe is opened and the bone spurs and part of the joint surface are removed (cheilectomy) or the base of the toe bone is resected (Keller), so the big toe can bend upward again without painful bone-on-bone impingement.

The case

Hallux rigidus is arthritis of the first metatarsophalangeal (MTP) joint, the joint at the base of the big toe. A bony spur grows on top of the metatarsal head, the toe loses its ability to bend upward, and walking and wearing shoes become painful. It is the second most common problem of this joint after bunions (hallux valgus). About half of patients do well without surgery, using shoe changes, stiff inserts, anti-inflammatory medicine or injections. The operation depends on how worn the joint is. For mild to moderate disease the usual choice is a cheilectomy: through a top or inner incision, the surgeon removes the spur and about a third of the top of the metatarsal head so the toe can bend up again. For severe disease, fusion of the joint (first MTP arthrodesis) is the standard; it has its own setup. For older patients who do not place high demands on the foot, the Keller resection arthroplasty removes the base of the proximal phalanx instead. Joint implants and spacer (interposition) procedures are other options that keep the joint moving. The setup is a small foot case with a thigh tourniquet, a minor orthopedic tray, small osteotomes and power for the drill and saw. Many patients walk in a surgical shoe right after a cheilectomy. A cheilectomy does not stop the arthritis from getting worse, and some patients later need a fusion. After a Keller, the toe can drift upward (cock-up toe), push-off can be weaker, and pain can move to the other metatarsal heads.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
