Proximal Hamstring Repair

The hamstring tendons torn off the ischial tuberosity (the sitting bone) are freed, pulled back down and reattached to roughened bone with suture anchors while the sciatic nerve next to them is protected.

The case

A proximal hamstring repair reattaches the hamstring tendons (semimembranosus, semitendinosus and biceps femoris) to the ischial tuberosity after they have been pulled off the bone. The injury usually happens with a forceful stretch while the hamstring is contracting, with the hip bent and the knee straight, and is common in athletes whose sport involves sudden speeding up and slowing down; the patient often feels a pop, then has bruising and pain in the back of the upper thigh. In children the same force can pull off a piece of bone (an avulsion fracture). Surgery is recommended for complete tears of all three tendons, for tears of two tendons pulled back more than 2 cm in young active patients, for bony avulsions displaced 2 cm or more, and for partial tears or tendinosis that do not improve with months of conservative care. Early repair is easier: after about four to six weeks, scar forms around the tendon and the sciatic nerve, the tendon is harder to bring down, and the risk of nerve injury rises. The open repair through a gluteal-crease incision is described here; it gives direct sight of the tendon and nerve and easy suture passing. An endoscopic repair through small portals with a 30-degree arthroscope is also done for less retracted tears, and some surgeons use a double-row repair with a knotless anchor. After surgery the knee is kept bent in a hinged brace (or the hip kept extended in an orthosis) for about six weeks, with return to sport usually at four months or later. Complications include wound problems and infection, numbness near the incision, sciatic or pudendal nerve irritation or injury, and re-rupture.

Before you ask

The honest answers.

What is CSTSetup?

A study site and apps for surgical techs and students, made by surgical techs. It has the real back table for 259 procedures, 2,882 instruments with photos, and practice for the CST, TS-C, CRCST, CSPDT and CSFA. Programs use it to teach and students use it to study. You can start free, and Pro opens all of it.

Can I cancel?

Any time, from your dashboard, and you keep Pro until the period you already paid for runs out. No email, no phone call, no retention script.

Does it work on my phone?

One account covers the website and the apps. Study on the bus, review on the unit, and your progress is the same everywhere. It lives on the account, not the device.

I haven't set an exam date yet.

You don't need one. Without a date the daily lesson is 10 questions built from what you are weakest on. Set a date later and it grows on its own as the date gets closer.

What happens to my progress if I stop paying?

Nothing is deleted. Your streak, your history and everything you have mastered are still there if you come back, and the free tier keeps working.

Is this enough on its own?

For the setups and instruments, yes: 259 trays and 2,882 instruments, tested until you are quick with them. For the exam it is practice, written to the exam's official outline, with full timed exams. It does not replace your program's textbook.
