What it means
The order in the chart that tells the team what to do in a cardiac or respiratory arrest. "Full code" means all resuscitation, including CPR, defibrillation and intubation. A do-not-resuscitate order (DNR, also DNAR or AND, allow natural death) means no CPR; the client still receives every other treatment and comfort care they want. The client or legal decision-maker sets it with the provider.
For the nurse
Know each client's code status at the start of every shift and check that the order, the armband (where used) and the handoff agree. Raise it with the provider if the client's wishes have changed.
In a chart
"Code status: DNR/DNI, confirmed with client and daughter; comfort measures continue."
