What it means
The Wong-Baker FACES Pain Rating Scale. Each face matches a number from 0 ("no hurt") to 10 ("hurts worst"). It is used for children about 3 years and older and for adults who find a number scale hard, such as those with limited English. The person chooses the face that matches how they feel, not how they look.
For the nurse
Explain each face in simple words before asking. Use the same scale each time so scores can be compared, and recheck pain after treatment.
