What it means
Used in notes to mean the client's past health story, as in "Hx of diabetes". It belongs to a family of short forms that end in x: Dx (diagnosis), Tx (treatment), Rx (prescription) and Fx (fracture).

Use only abbreviations on your facility's approved list.
In a chart
"Hx of HTN and type 2 diabetes; no known drug allergies."
