What it means
A cycle of five steps. The nurse collects data (assessuh-SESTo gather information about a client by looking, listening, asking, measuring and examining.Full entry ), decides what problems the client has (nursing diagnosis), sets goals and chooses actions (planning), carries out the actions (implementation) and checks whether the goals were met (evaluateih-VAL-yoo-aytTo judge whether care worked by comparing the client's results with the goals that were set.Full entry ). The cycle repeats as the client's condition changes.
For the nurse
Exam questions often test the order: assessment comes before action unless the danger is already clear.
