patient-centered care

Care planned around the client's own needs, values and choices, with the client as a full partner in decisions.

PAY-shunt SEN-terd KAIR noun

What it means

An approach in which the client, and the family if the client wishes, help decide the plan of care. The team respects the client's culture, beliefs and preferences, shares information openly, and supports comfort and emotional needs. It is one of the core competencies taught to nurses (QSEN).

For the nurse

Ask what matters most to the client and include it in the plan. Explain options in plain words and check understanding before decisions are made.
