What it means
On the NCLEX-PN test plan, LPN/VNs "reinforce" teaching that the RN or provider began: they go over it again, answer questions and correct mistakes. The RN is responsible for the first teaching and the teaching plan. Reinforce can also mean making a dressing stronger by adding layers over it without removing it.
For the nurse
When a client's statement shows a misunderstanding, reinforce the correct information and report to the RN if new teaching is needed. A dressing that bleeds through may be reinforced per orders, and the amount of drainage is noted and reported.
