What it means
A physical restraint is a device such as wrist ties, a vest or, in some cases, four side rails up. A chemical restraint is a drug given to control behavior that is not a standard treatment for the client's condition. Restraints are a last resort, used only to keep the client or others safe when less restrictive measures have failed, and need a provider's order for a limited time.
For the nurse
Try alternatives first (sitter, frequent rounding, bed alarm, treating pain or confusion). If used, check the client and restraint often per policy, offer food, fluids and toileting, release for range of motion, and tie to the bed frame with a quick-release knot, never to the side rail. Document the reason, alternatives tried and each check.
