durable power of attorney

A legal document in which a person names someone to make decisions for them if they become unable to decide for themselves.

DOOR-uh-bul POW-er uv uh-TER-nee noun

What it means

"Durable" means it stays in effect even after the person loses the ability to make decisions. A durable power of attorney for health care names an agent (also called a health care proxy) to make medical decisions; a separate financial one covers money and property. It is one type of advance directive.

For the nurse

Ask on admission whether the client has one and keep a copy in the record. The agent can decide only when the client cannot; while the client is able, the nurse takes decisions and consent from the client.
