What it means
A structured way to report, for example when calling a provider about a change in a client. The nurse says who they are (Introduction), what is happening now (Situation), the relevant history (Background), what they found and think (Assessment), what they need (Request or Recommendation), and then repeats back any orders received (Repeat back). It grows out of the shorter SBAR.
For the nurse
Have the chart, recent vital signs, labs and drug list in front of you before you call, and read back every order.
