letdown

The reflex that pushes milk out of the breast's milk glands toward the nipple during breastfeeding.

LET-down noun

What it means

When a baby sucks, the pituitary gland releases the hormone oxytocinok-sih-TOH-sinA hormone that makes the uterus contract and releases breast milk; also given by IV to start or strengthen labor and control bleeding after birth.Full entry . Oxytocin makes the small muscle cells around the milk sacs squeeze, and milk flows to the nipple. A parent may feel tingling or fullness, milk may drip from the other breast, and in the first days the uterus cramps at the same time. A baby's cry, or even thinking about the baby, can start it; pain, stress and tiredness can slow it.

For the nurse

Help the parent get comfortable and relaxed before feeds, and treat pain. Teach that cramping during feeds in the first days is expected and helps the uterus shrink back.

In a chart

"Mother reports tingling and milk flow within two minutes of latch; strong letdown."
