What it means
In charting, PMH is the client's past medical history: earlier illnesses, surgeries, hospital stays and long-term conditions. In nursing roles it also appears in titles such as PMHNP, a psychiatric-mental health nurse practitioner, and PMH-BC, a board certification for psychiatric-mental health nurses.
