Transvenous lead extraction. If simple traction with a standard stylet fails, the lead is cut behind the connector and the locking stylet is put in so pulling force reaches the lead tip. It keeps the lead under tension while extraction sheaths are advanced over it, and allows countertraction at the heart wall.
