A hydrocele is a collection of fluid around a testicle, inside the thin sac (the tunica vaginalis) that surrounds it in the scrotum. It makes that side of the scrotum swell. In adults it can follow an injury or an infection. In children a hydrocele may come with an inguinal hernia, and then it is repaired through a cut in the groin together with the hernia.
Draining the fluid with a needle usually helps only for a while, because the fluid comes back. A hydrocelectomy fixes it with an operation. Through a small cut in the front of the scrotum, the surgeon frees the sac and empties the fluid, then cuts away most of the sac, or turns it inside out and stitches it behind the testicle (sometimes called bottling the testicle), so there is no closed space left for fluid to fill. The testicle itself is not removed. A small drain may be left in.
Adults usually have numbing medicine with sedation, and children have general anesthesia. Most adults go home the same day; a child may stay overnight. A bulky gauze dressing in a scrotal support keeps light pressure on the area to limit swelling. Most people are back to normal activities in 7 to 10 days. The main problems are swelling of the scrotum and wound infection.
