How do I get the NCCT case verification form for a student?

Open the student's record in your portal and choose Export. We fill the official 2025 NCCT forms for you: TE-0204CSQE (the student's form) and TE-0204CSQE-INS (the instructor's form), as real PDF forms with the fields completed, ready to sign and submit.

What goes on the form

Only approved cases. The scrub-role and category counts match exactly what the student's progress page shows, so what you sign is what they see.

The numbers don't match what I expected

Check the Review queue for cases still pending; they aren't counted until approved. And remember that under AST, observation cases don't count at all, while NCCT and CC4SA include them. See how the role is set.

AST and CC4SA programs

There is no official form to fill for those standards. The same export gives you the student's case data as a printable summary you can attach to your own paperwork.
